Add system restrictions to disallow duplicate extension number assignment.

The system should never allow an extension number to be assigned to more than one user, AA, HG, etc. Yet currently this occurs on the system. Ideally, the system should deny the assignment and inform the user that the number is already in-use.

Dennis, with the release of OSSmosis 5, there is a validation check in place where users will not be able to use a previously used extension for their USER ID, HG, AA, CC. So for all NEW clients or builds that functionality is in place. Unfortunately older clients may have overlapping extensions, especially those with 100s of locations.
